Eagle's Nest School Based Health Center (SBHC)

Mission
To promote quality healthcare to our youth by providing services and programs in school that assure that they are physically and emotionally prepared to take full advantage of their educational opportunities and experience a positive, healthy lifestyle.

SBHC - How It Works
This program supplements the traditional school nurse services. As always, there are no charges associated with school nurse visits. The nurse practitioner and the school nurse work closely together to provide the best service to students.

The School-Based Health Center is located in the Nurse's Office and staffed by an administrative assistant. The nurse practitioner, who sees patients three days per week, is also available by phone if circumstances warrant.

Enrollment forms and information release forms are sent home on the first day of school. We accept new members throughout the school year. Forms are available for download at the bottom of this page, in the School-Based Health Center, Nurse's Office, SAO and Guidance Department.

Appointments can be made directly with the administrative assistant or through the school nurse. Every effort will be made to schedule all appointments during study halls or during a time that least impacts the student's schedule.


All mental health services are provided through a cooperative contract with Sweetser, Maine's most comprehensive behavioral health care organization for children, adults, and families. Separate forms for consent and insurance information are required. Sweetser does bill insurance and does require co-pays if required by your insurance plan.


Services

  • Diagnosis and treatment of acute illnesses (sore throat, bronchitis, injuries, rashes, etc.)
  • Onsite lab tests for strep throat, urine tests
  • Management of chronic conditions (asthma, diabetes, headaches, acne, etc.)
  • School sports and camp physicals
  • Coordination of care with student's primary care provider
  • Tobacco cessation/quit smoking, drug, alcohol referrals available
  • Alcohol and substance abuse referrals
  • Counseling services
  • Information on acquiring MaineCare
  • Assistance in finding a primary care provider if needed


Eligibility
With permission of parent or guardian, students of Mt. Ararat High School are eligible. Students 18 years or older can sign themselves up for the SBHC.


Hours
The nurse practitioner is available: Monday, Tuesday and Thursday - 7:30am - 2:00pm.


Payment/Billing
This is a no cost service.

Your child has two options for health care at MTA:

 

Option One - Nurse's Office

 

Option Two - School Based Health Center

School Nurse (RN) School Nurse plus SBHC Nurse Practitioner (NP)
  • Health care for all students
  • No enrollment necessary
  • Enhanced services
  • Student Must Be Enrolled
 
  • Care provided by School Nurse
  • Same kind of care as provided in grades K-8
  • Create and follow health care plans for students with chronic conditions
  • Can only dispense medication with doctor’s order and parent permission (this includes over the counter medications such as Tylenol and cough syrup)
  • Includes all benefits of Option 1, plus additional care provided by NP.
  • NP can diagnose health conditions and prescribe medication. Student does not have to leave school to see a doctor for health issues such as Strep, Mono, cold symptoms, strains and sprains.
  • RN and NP can dispense medication as deemed medically necessary such as Tylenol and Ibuprofen.
  • Every student enrolled receives a comprehensive Health Assessment.
  • Saves trips to the doctor, but does not take the doctor’s place – NP works with the doctor to care for ongoing health concerns.
  • Keeps students engaged in maintaining their overall health and wellness.

Parents/guardians are very welcome to call the School-Based Health Center or the school nurse with questions or concerns about their child's health.

Maine state laws regarding parental consent for care are the same as they are at your doctor's office; parental consent is not required for the treatment of drug abuse, pregnancy testing, and testing for and treating sexually transmitted diseases.
